Step 1: Mental Health Rights Under National Health Insurance
Since July 2015, mental health care in Israel has been covered under the National Health Insurance Law (chok bituach briut mamlachti, חוק ביטוח בריאות ממלכתי). This landmark reform transferred psychiatric and psychological services from the Ministry of Health to the kupot cholim (health funds).

2025-2026 system status: the government approved adding approximately 366 million NIS to the 2025 health basket and an additional 175 million NIS in 2026 to expand mental health services. Kupat cholim mental health budgets jumped from around 347 million NIS in 2024 to around 591 million NIS in 2025. Despite this, demand continues to outpace supply and waiting lists remain a known bottleneck.
Step 2: Accessing Therapy Through Kupat Cholim
Each of the four kupot cholim provides mental health services. The process for starting therapy is as follows:

Important notes:
- Wait times can be 2-8 weeks (and longer in many regions since 2023). The 2025 Ministry of Health funding report shows treatment volume grew substantially but demand still outpaces supply.
- Kupot typically offer 12-16 individual sessions per year per patient before review
- If the assigned therapist is not a good fit, you have the right to request a different one
- Supplementary insurance (shaban, שב"ן) may provide additional sessions or faster access
- Children and adolescents often receive priority placement
- When wait times exceed reasonable thresholds, ask about external therapist reimbursement (shlichut, שליחות). Kupot are obligated to provide an alternative path under the National Health Insurance Law.

When to use crisis hotlines:

Step 4: Types of Treatment Available
Israeli mental health services offer several evidence-based treatment modalities.

Post-October 7 expansion (still active 2025-2026):
- EMDR services were significantly expanded across all kupot cholim
- Additional trauma-focused therapists were trained and deployed
- Group therapy programs for communities affected by the events were established
- Resilience centers (merkazei chosen, מרכזי חוסן) operated by the Ministry of Health in the south, north, Judea and Samaria, plus a national resilience center created after October 7. Together they serve roughly 1.3 million residents (per State Comptroller 2025 report)
- Sal briut 2026 added two psychiatric medications for adults with major depression: Auvelity (rapid-acting NMDA-targeting agent) and Riagility (dopamine-receptor agent helpful for negative symptoms). Always verify current sal briut coverage with your kupat cholim pharmacist.
Step 5: Private Therapy Options and Costs
For those who prefer private therapy or cannot wait for kupat cholim placement. The 2025 HebPsy rate survey put the national average at around 395 NIS per session (about 2% higher than 2024), with the Tel Aviv / Shfela area roughly 40 NIS above the periphery.

Step 6: Workplace Mental Health
Israeli labor law provides protections for employees dealing with mental health challenges.

Step 7: PTSD-Specific Resources
PTSD (post-traumatic stress disorder, הפרעת דחק פוסט-טראומטית) is widespread in Israel. The Ministry of Defense Rehabilitation Division reported in late 2025 that it is handling around 82,400 wounded, of whom about 31,000 are dealing with PTSD or other mental health conditions, with around 1,500 new recognition requests per month and an expected additional 10,000 recognitions by end of 2026.

Bituach Leumi general disability recognition (mental health track):
- Complete Form 7801 (tviya le'kitzbat nechut klalit, תביעה לקצבת נכות כללית) online or at a local branch
- Attach a psychiatric or psychological opinion plus relevant medical documentation
- Attend a medical committee evaluation (vaada refuit, ועדה רפואית)
- Eligibility requires medical disability of at least 60%, or at least 40% if there are multiple impairments and one of them reaches 25%
- As of January 2026, the full single pension (kitzva yechid male'a, קצבת יחיד מלאה) is around 4,711 NIS per month for those with at least 75% loss of earning capacity, with earnings tested against a monthly threshold of around 8,261 NIS
- Recognized status entitles the person to additional subsidized treatment and rehabilitation services via the mental health rehabilitation basket (sal shikum, סל שיקום)
For terror or war-related trauma, the hostility-victims track at Bituach Leumi (medical/financial benefits via the Ministry of Defense determining authority) is usually faster and more generous than the general disability track. Use Form 571 for Iron Swords claims.
Post-October 7 programs (still active 2025-2026):
- Extended PTSD treatment coverage through all kupot cholim
- Emergency mental health response teams deployed to affected communities
- EMDR training programs expanded for therapists nationwide
- Digital mental health platforms made available at no cost
- Reservists who served 30+ days under tzav 8 (order 8) since January 1, 2025 are eligible for reimbursed psychological, emotional, and couples therapy for themselves and for their spouses and children up to age 14
- Late 2025 policy clarification by Bituach Leumi recognizes that PTSD is not necessarily a time-limited condition, and additional treatment series for recognized hostility victims are approved with greater flexibility

Example 1: Starting Therapy for Anxiety
User says: "I've been having panic attacks and want to start therapy. How do I do this through my kupat cholim?"
Actions:
- Call your kupat cholim's mental health department (machleket briut hanefesh)
- Request an intake appointment; describe your symptoms (panic attacks, anxiety)
- You will be assigned to a therapist, likely a CBT practitioner for anxiety
- Cost will be approximately 33 NIS per quarter for psychiatrist visits, or free for psychologist/social worker visits at the clinic, through the subsidized program
- If wait times exceed 4 weeks, ask about external therapist reimbursement options
Result: Patient begins weekly CBT sessions through their kupat cholim. Sessions are subsidized under the 2015 mental health reform. If the assigned therapist is not a good match, the patient can request reassignment.
Example 2: Crisis Support After a Difficult Event
User says: "My friend just told me they're having suicidal thoughts. What do I do right now?"
Actions:
- Stay with your friend; do not leave them alone
- Call ERAN at 1201 together, or call on their behalf
- If there is immediate danger, call 101 (MDA) or 100 (police)
- Encourage your friend to go to the nearest hospital ER psychiatric emergency (cheder miyun psikh'iatri)
- After the immediate crisis, help them schedule an appointment with their kupat cholim
Result: ERAN provides immediate crisis intervention and guidance. If hospitalization is needed, the ER psychiatric team evaluates and can arrange voluntary or involuntary admission for safety.
Example 3: Navigating Workplace Mental Health Leave
User says: "I need to take time off work for depression but I'm worried about my job. What are my rights?"
Actions:
- Visit your psychiatrist or family doctor to obtain a sick note (ishur machala)
- The sick note does not need to specify "depression" or any diagnosis
- Submit the sick note to your employer's HR department
- You are entitled to 1.5 sick days per month (18 per year)
- Your employer cannot fire you during sick leave or discriminate based on your condition
- If you need extended leave, discuss with your doctor about ishur machala for a longer period
Result: Employee takes mental health sick leave. Employer receives a generic sick note without diagnosis. Employment is protected under Israeli labor law
